About the Position
We are currently working virtually in this Navos position until the pandemic is brought under control. The people we serve are older adults who struggle with severe and persistent mental illness. Typically they have a number of chronic health conditions, live in near poverty, and are too often isolated from their community and family. Thus, we have a great opportunity to make a significant difference in their lives by providing case management and therapy in a recovery environment. This is an opportunity for new clinicians and seasoned clinicians alike to learn more, grow, help clients prosper, and to work in a supportive environment.

Position Functions & Responsibilities
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. • Provide case management services to 60 older adult clients living in the community;
Current Case Managers will be working remotely using telehealth until otherwise noted
• Develop individualized treatment plans;
• Secure needed resources;
• Educate clients about mental illness;
• Coordinate with family members and other professionals;
• Re-teach independent living skills;
• Conduct diagnostic interviews;
• Assure ongoing medical treatment;
• Provide transportation as necessary;
• Monitor for decompensation;
• Provide crisis services; and Coordinate with other service providers.

Position Requirements
• This position requires a Master’s Degree (MA / MC / MS / MSW) in human services

Preferred Qualifications
Knowledge, Skills, & Abilities, Education and/or Experience
• Demonstrated knowledge of older adults, mental illness, managed care system, and community resources.
• Ability to provide intakes and make diagnoses per the DSM V.
• Strong organizational and communication skills.
• A Washington State counselor registration or license.
• A valid WA State driver's license and the ability to use one’s own car when needed

Supervisory Responsibilities:
This position has no supervisory responsibilities

Physical Demands:
Able to lift at least 10lbs; able to sit for long periods of time; some bending, stooping may be required.

Work Environment:
The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. While performing the duties of this job, the employee is at risk for exposure to blood borne pathogens. Employee must follow organization infection control policies. The noise level in the work environment is can be loud in the waiting areas.
