Position Summary:
The Clinical Ortho-Surgical Program Coordinator is responsible for the clinical, quality, and cost effective outcomes for orthopedic patients at our local Hospitals and Ambulatory Surgical Centers from initial contact throughout the continuum of the admission through coordination of an interdisciplinary plan of care. The Coordinator acts as an expert clinical practitioner, mobilizes resources and manages the system in response to patient needs. He/she facilitates pre-operative, post-operative coordination of care, provides education and needs assessments to prepare the post-operative patient for transition to the anticipated post-hospital care setting and guides the patient progress toward achieving clinical, quality, and cost effective outcomes. The Coordinator participates in the development and evaluation of clinical guidelines or standards of care, collects, trends, and analyzes aggregate data for an identified population of patients to be used in identifying best practices and performance improvement efforts.

Minimum Requirements:
Five (5) years case management or equivalent experience

Bachelor’s degree in nursing required
Master’s degree in nursing or healthcare-related/administrative field- preferred
Licensed as a registered nurse in the State of Washington
Case Manager Certification preferred
