Position Summary:
The Director of Population Health Management  primary focus is to improve the overall health status of a given population while lowering the cost of care.  The Director oversees staff responsible for patient care coordination in the ambulatory setting and evaluates patient data to ensure that the program is being executed in a manner that appropriately supports all acuity levels of patients.  The Director directs the team that works in collaboration and continuous partnership with chronically ill or “high-risk” patients and their family/caregiver(s), clinic/hospital/specialty providers and staff, and community resources. The Director works closely with MultiCare Connected Care, Risk Insurers, ACO partners, Utilization Management, system and hospital leaders, physicians, peers, community colleagues, and others to identify opportunities to continually improve patient care and services.

Minimum Requirements:
Eight years clinical experience
Five years of healthcare leadership experience
Risk mitigation, financial and reimbursement experience

Masters degree in health related field
Current Washington State RN Licensure
