The ideal candidate for the Manager of Patient Navigation and Care Coordination is responsible for day to day leadership activities managing a team of clinical professionals in the execution of exemplary cross continuum care coordination. This individual works with the director, market leaders, physicians, peers, community colleagues, and others to identify opportunities to continually improve patient care and services, and to execute collaborative practices that exemplify MultiCare Health System’s commitment to patient centered care and community engagement.
Three to five years leadership experience and demonstrated skills in progressive management roles
Three years of healthcare leadership experience
Clinical experience a plus
Consideration may be given to internal candidates not meeting the minimum qualifications
Masters Degree in health related field
Licensed in the State of Washington as a RN or Licensed Independent Clinical Social Worker (LICSW)
