Responsible for working with the pharmacy/infusion staff to provide optimal pharmaceutical care for patients. Responsible for patient assessment, development of patient specific care plans, dispensing of medications, participation in patient and staff education, and consultation with other professionals. This position operates independently with limited supervision. Work situations require precision, attention to detail, a service-oriented individual, and discretion and good judgment in decision-making. Internal contacts are primarily with nursing staff, pharmacy, dietary, and management.
Minimum three years hospital or home heath pharmacy experience required.
Graduate of an accredited College or School of Pharmacy. Licensed as a pharmacist in the State of Washington. Completion of an ASHP accredited pharmacy residency preferred. Current CPR certification.
