Position Summary: Performs Medicare Annual Wellness visits, and coordinates follow-up chronic illness reviews appointments with primary care providers – in adherence to Medicare Risk Adjustment contracts.
Teaches prevention, safety and health issues through discussion with patients.  Places orders per protocol, performs point of care testing and document visits. Maintains schedule and caseload of Medicare Advantage patients at multiple Rockwood Clinic locations.
Works collaboratively with assigned physician and mid-level providers, and is an integral member of the health care team – ensuing safety, best practices and high quality standards of patient care.  
Works closely with the Rockwood QI Department and medical directors to assess and improve results. Success will be measured by the results of process and outcome performance measures.

Minimum Requirements: Minimum of one (1) year professional nursing experience in a clinical setting preferred         
Education/Licenses/Certifications:High School diploma or General Education Degree (GED)         
Current Registered Nurse license in Washington State (and/or Idaho, if clinic is in Idaho)         
Current CPR/BLS certification required
