The Social Worker participates as a member of an interdisciplinary health care team to identify patient’s psychosocial needs and to develop treatment and discharge plans to ensure a patient’s illness, recovery and safe transition from one care setting to another are considered within the context of their bio-psychosocial needs. The incumbent also provides therapeutic interventions to individual patients and their families; provides treatment groups for patients and/or support groups for caregivers; collaborates with other social service provider agencies; links individuals to resources; helps restore individuals, families, and groups to successful social functioning; serves as an advocate for the patient and, when required, an advocate for broader social causes.
Five years experience required (hospital/behavioral health setting preferred)
Prior experience collaborating on needs assessments, service planning, interagency follow-through and/or case management is required (health care setting preferred)
Master’s Degree in Social Work required
Licensed in the state of Washington as a Licensed Advanced Social Worker (LASW), Licensed Independent Clinical Social Worker (LICSW), Licensed Social Worker Associate Advanced (LSWAA), or Social Worker Associate Independent Clinical (LSWAIC); or commitment to obtain within 24 months of hire
This position will help cover both Adult and Geriatric Psych units
