Position Summary:
The Triage & Transfer Coord assists providers and/or staff in the management of telehealth, community and system resources, and access to high quality care. The work includes providing a high level of telephonic and administrative support, program coordination and data management functions for a multidisciplinary, collaborative practice environment in keeping with department policy and regulations.  The Triage & Transfer Coordinator provides excellent customer service to ensure patient and customer care needs are met. The incumbent has contact with patients, patient families/significant others, community agencies, providers, staff and management when advocating for patients. Work situations are highly varied with rapidly changing priorities, which require strong interpersonal and organizational skills, diplomacy, decision making, knowledge and integration of available standards, resources and data.

Minimum Requirements:
Two (2) years experience in the production of highly technical documents preferred
Previous experience working in emergency/crisis situations preferred

High school graduate or equivalent plus two (2) years of clerical experience using a multi-line telephone system in a high volume setting that would provide the requisite knowledge and skills required; education beyond high school may be considered in lieu of one of the two years of directly applicable work experience at the discretion of management and Human Potential.
Current license as an LPN in the state of Washington preferred

Must be available to attend eight consecutive days of general orientation upon start.
